Previous incidents

All forms are back to their working states. Clients making form submissions and bookings between 00:45 and 01:10 UTC may have encountered issues and will need to resubmit if they were displayed with an error message instead of a confirmation screen.

We're identified the underlying issue and a patch has been deployed. All forms are now fully functional.
